New Patients

If you are new to New Frontiers Health Services, we ask:

-You have your medical records transferred from your previous providers office to our office prior to your appointment. Having your records available prior to your appointment helps our providers be able to know more about your medical history. Records can be faxed, mailed or hand delivered to our office. Our fax number is 785-672-8194.

- Please bring your insurance and identification cards to your appointment. This allows us to verify your information.

- If you are taking any medications (prescription and non-prescription), please bring these or a list of these to your appointment.

- Please have a current copy of your child’s immunization records for their appointment.
