It is important to us that we provide guidance and support to ease the stressors related to transition of care and our Social Service Designee acts as our liaison between community, families, and staff. Our designee is the primary contact during the admission process for the purpose of providing the necessary emotional support during this transition. She offers one-on-one support and encouragement and assists in identifying strengths and needs to promote a more person-centered environment.
