APPLICATION FOR PATIENT AND FAMILY ADVISORS

Wanted !!!

We are looking for an enthusiastic, diverse group of individuals who are interested in participating in our Patient, Resident, and Family Advisory Council (PRFAC).  Evidence has shown that there are better patient outcomes and a more patient centered culture when patients and families are involved in the care and experience of the patient or loved one.  PRFAC councils may help with improving processes, communication, flow of care and/or facility lay out.

If you are interested please fill out an application and mail it in or return it to the hospital.  We are taking applications through the end of August and we plan to have our first meeting by the end of September.
