It is important to us that we provide guidance and support to ease the stressors related to transition of care and our Social Worker acts as our liaison between community, families, and staff.  Our designee is the primary contact during the admission process for the purpose of providing the necessary emotional support during this transition.  She offers one-on-one support and encouragement and assists in identifying strengths and needs to promote a more person-centered environment.

 

Skilled Swingbed Services

Skilled Swingbed is an extended care service provided to Medicare patients that no longer require acute hospitalization, but are not able to go home and care for themselves.

Rehabilitation (PT/OT/Speech) is an essential part of Skilled Swingbed care. Therapists work with you on strength and mobility so you can progress towards independence.

Activities of daily living (i.e. dressing, bathing, walking, etc.) are an important part of this independence. As a Skilled Swingbed patient, you will be encouraged to help bathe and dress yourself. To achieve this, we encourage families to bring in your clothing and personal care items.

Our team will work with you, your family or your caregiver to ensure a smooth and safe transition to a prior or new living environment.

 

To be eligible for Skilled Swingbed Services, these conditions must be met:

  • Must have an order from the Medical Provider.
  • Must have 3 consecutive midnights in acute care.
  • Services must begin within 30 days of acute stay.
  • Care needs must warrant skilled care based on a needs assessment by the care plan team.

Your Care Plan Team consists of:

  • Medical Provider
  • Swingbed Coordinator
  • Director of Nursing
  • Dietary Staff
  • Nursing Staff
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Respiratory Therapy
  • You & Your family

Your care plan team meets every Wednesday to discuss how we can best meet your health care needs. This meeting allows a dedicated time to collaborate on treatments, needs, accomplishments and challenges. We encourage you and your family to attend. Special family meetings can be arranged with the Swing Bed Coordinator.
